Applications on prescribed forms are invited from eligible Pakistani National candidates for Postgraduate Training of FCPS Level-III & IV Session July 2023 at Shaikh Zayed Federal Postgraduate Medical Institute, Lahore detail is as under

3. FCPS HPB / LIVER TRANSPLANT SURGERY (subject to approval from CPSP) 
4. FCPS PAIN MEDICINE 
1. Basic Criteria of Level-Ill Training Disciplines 
2. Must possess Level-Ill FCPS Degree in Medicine/Surgery and Anaesthesia respectively.

ELIGIBILITY CRITERIA

1. Basic Medical Quairficaton of MBBS or equivalent medical § qualification recognized by the PMDC/PMC
2. Certificate of one-year house job in an institution recognized by the PMDC/PMC
3. A valid certificate of permanent registration with the PMDC/PMC
4. FCPS Part-l Congratulations letter 
5. Candidates can also apply for Level-3 Year-3 in Surgery & Medicine Allied Departments. 

REQUIRED DOCUMENTS

• Matric & intermediate Certificate
• MBBS degree & detailed marks sheets of all professional examinations
• FCPS Part-I passing certificate/result of above cited CPSP programs accordingly 
• PM&DC valid permanent registration Certificate » 
• House Job certificate with exact dates from the concerned Medical superintendent/Administrators. 
• distinction Certificate.  
• Copy of publications (if any) in approved journals from PM&DC/PMC/PubMed Indexed & HEC
• Domicile Certificate 
• Computenzed National Identity Card (CNIC)
• Only Relevant subject/specialty-related experience certificate from Teaching Hospital recognized
• One passport-size photograph duly attested from the back side 
• Those having foreign qualifications are required to submit HEC/PMOC equivalency certificate

• processing & application fee (Rs 2000) shall be submitted through Intra Bank Transection (IBT) in favor of Deputy Dean Shaikh Zayed Postgraduate Medical Institute, Lahore Account No 3154820902 National Bank of Pakistan (NBP) Shaikh Zayed Hospital, Lahore Branch code 1707 
• and forms can be sent through couriers to the above-cited form address. 
• Copy of Intra Bank Transecthon must be attached with application form Forms are also available from the account section of FPGMI, Lahore on Payment of Rs. 2000/
• Bank Draft or Pay Order not accepted
• The information in the application form shall be verified at three (03) stages
• During the submission & processing of the application 
• At the time of Acceptance/joining
• During the training 
• If the information submitted was found wrong, mismatched, or forged. the application will be rejected and the contract will be discontinued with no right of appeal at the institution/government level
• Applications received after the due date or with incomplete information (Academic Info form) should not be entertained
